Abstract     The administration of interferons in transplant recipients has areoused concern about possible side effects. Recently, we have had the opprtunity to study a patient who, after a long period of renal graft stability, developed immunosuppressive therapy resistant rapidly progressive deterioration of graft function associated with the application of INF- alpha due to chronic myelogenous leukemia (CML). Widespread interstitial edema, found by percutaneous needle biopsy, caused problems of interpretation, whereas the explanted graft study clearly demonstrated severe acute, progressive, predominantly vascular rejection. We could not find a single similar case report in the available literature on IFN-alpha induced renal failure in which biopsy findings were accomplished substantially by the study of subsequently explanted kidney and appertaining extrarenal vessels.
