Abstract     Background: Allergic reaction to stinging insects is one of the more dramatic allergic diseases. The preventiv etreatment prsently available is immunotherapy. The aim of this study was to analyse results of immunotherapy in our patients. Materials and methods: We retrospectively analysed data from 128 children, who had undergone Hymenoptera venom rush immunotherapy because of an allergic reaction after a bee (81 patients) or wasp (47 patients) sting at the Paediatric University Hospital during the period from 1985 to 1996. We were interested in the decrease of specific serum IgE antibodiy values after 5 years' immunotherapy in comparision with initial values, and levels of venom specific IgG antibodies after 5 years of immunotherapy, adverse effects during immunotherapy and outcome after accidental subsequent field re-sting. Results: We found a decrease in specific serum IgE antibodies in 70.4% patients in bee venom immunotherapy and 12.7% patients on wasp venom therapy. Levels of venom specific IgG antidobies were sufficient in 66.6% patients on bee venom immunotherapy and 61.7% patients on wasp venom immunotherapyafter 5 years of treatment. 34.6% of patients on bee venom immunotherapy and 29.8% of patients on waslp venom immunotherapy had adverse effects, most of which were local reactions. 66.6% of patients on bee venom immunotherapy and 55.3% of patients on wasp venom immunotherapy were re-stung. 92% showed no reaction or only a local one. Conclusion: Our results showed, that immunotherapy was very successful with only mild reactions after field re-sting during immunotherapy, and rare adverse effects. Levels of specific IgE serum antibodies and venom specific IgG antibodies in our patients were of low value in predicting the effect of immunotherapy.
Summary     Izhodišča: Alergijske reakcije po pikih insektov so med najbolj dramatičnimi. Preventivna metoda, ki je trenutno najuspešnejša, je imunoterapija. Cilj pričujoče študije je bil,ugotoviti rezultate imunoterapije na strup insektov pri naših bolnikih. Preiskovanci in metode: retrospektivno smo analizirali podatke o 128 otrocih, ki jih z metodo imunoterapije (81 na strup čebele, 47 na strup ose) zdravimo na naši kliniki v času od leta 1985 do 1996 Zbirali smo naslednje podatke: znižanje kocentracije specifičnih IgE po petih letih imunoterapije v primerjavi z izhodnimi vrednostmi; raven specifičnih serumskih blokirajočih IgG po petih letih imunoterapije; zaplete ob imunoterapiji in reakcije ob ponovnem piku insekta. Rezultati: Ugotovili smo padec specifičnih IgE protiteles na strup čebele pri 70,4% bolnikov na imunoterapiji s strupom čebele in pri 12,7% bolnikov na imunoterapiji s strupom ose. Raven specifičnih IgG protiteles je bila v zadovoljivem območju pri 66,6% bolnikov na imunoterapiji s strupom čebele in pri 61,7% bolnikov na imunoterapiji s strupom ose. Zaplete ob imunoterapiji s strupom čebele smo zasledili pri 34,6%, pri imunoterapiji s strupom ose pa pri 29,8%. Večinoma je šlo za lokalno reakcijo. Čebela je pičila 66,6% bolnikov na imunoterapiji s strupom čebele, osa pa 55,3% bolnikov na imunoterapiji s strupom ose. V 92% po piku ni bilo reakcije ali pa je bila lokalna. Zaključek: Imunoterapija na strupe insektov je zelo uspešna, saj ob ponovnem piku insekta med potekom imunoterapije ni alergijske reakcije ali pa je le lokalna. redki in blagi so tudi zapleti, ki spremljajo imunoterapijo. laboratorijski rezultati specifičnih IgE in specifičnih IgG na strup insektov pa so pokazali slabo napovedno vrednost za učinek imunoterapije.
