Abstract		Haematopoietic stem cell transplantation has achieved significant therapeutic success for many previously incurable disorders. Due to the donor's CD4+ T-cells in the graft, the procedure may be compromised by acute and chronic graft versus host diseases. Various treatment modalities can decrease the intensity of these reactions, but unfortunately also decrease the graft versus leukemia effect, which is very important for complete eradication of residual disease. Besidesthe well known early complications, there has been much data in the last ten years about late effects of stem cell transplantation, which may have an impact on the long-term success of the transplantation.
Summary		Presaditev krvotvornih matičnih celic je sodobno in zelo uspešno zdravljenje cele vrste poprej neozdravljivih bolezni. Ker gre poleg presaditve pluripotetne matične celice tudi za presaditev imunokompetentnih celic, predvsem limfocitov T CD4+, prihaja poleg drugih zapletov do neravnovesja v imunskem stanju bolnika. Najočitnejši klinični znak tega sta akutna in kronična reakcija presadka proti gostitelju, ki lahko ogrozita bolnikovo življenje. Z intenzivnimi načini preprečevanja teh reakcij sicer zmanjšamo njihovo intenzivnost, vendar ob tem zmanjšamo tudi sočasno reakcijo presadka proti levkemiji, ki je bistvenega pomena za uspešno zdravljenje nekaterih bolezni. Poleg neposrednih zapletov pa v zadnjih letih ugotavljajo vrsto poznih zapletov, ki lahko bistveno vplivajo na dolgoročno uspešnost tega zdravljenja.
