Abstract     Background. Mechanisms of osteochondral (OH) fractures of the knee caused by shearing forces between the femur and tibia due to indirect violence have received little attention in the literature so far. We present a patient whose fractured OH fragment was reduced and fixed using our original arthroscopic technique. It provides exact reposition and reliable fixation even of smaller fragments, and thereby prevents the development of degnerative arthritis and problems related to the presence of loose bodies in the joint. Conclusion. Mechanical stabilisation of an OH injury in an acceptable position increases the likelihood of satisfactory healing by restoring congruity of the articular cartilage.
Summary     izhodišče. Mehanizem osteohondralnega (OH) zloma v kolenu zaradi strižnih sil med stegnenico in golenico kot posledica posredne obremenitve se v literaturi redko omenja. OH fragment smo reponirali in fiksirali z izvirno artroskopsko metodo, ki omogoča natančno repozicijo in zanesljivo fiksiranje tudi manjših fragmentov. S tem preprečimo razvoj degenerativnega artritisa in problemov prostih teles v sklepu. Zaključek. Mehanična stabilizacija OH zloma v zadovoljivem položaju in vzpostavitev kongruence sklepnega hrustanca poveča verjetnost uspešnega zdravljenja.
