Abstract     Atrial septal defect (ASD) secundum is one of the most frequent congenital heart diseases. Haemodynamicly important ASD secundum does not have very well long term prognosis. In adult population, ASD may cause heart failure, hear arrhytmias, thrombembolic events, pulmonary arterial hypertension and early death. The ratio between pulmonary and systemic flow rates (Qp:Qs) is one of the most important indexes in evaluating the haemodynamic importance of ASD secundum. In the research project I make a new protocol for echocardiographic calculation of Qp:Qs using maximal flow velocity through the mitral and tricuspid valves (Qp:QsV). I examine 36 patients with ASD secundum and calculate theirs Qp:Qs using new method, scintigraphic method and standard echocardiographic method in which we use average flow velocities through valves. I found out that new method excellent correlates with scintigraphic calculation of Qp:Qs (r = 0.96, p<0.01) and regression equation is Qp:Qs = 0.3 + 0.8 X Qp:QsV. At the same time, I show that method correlates with scintigraphic Qp:Qs statistically important better than echocardiographic calculation of Qp:Qs using average flow velocity through valves.
