Abstract     Background. Contamination of specimens from the lower respiratory tract (LRT) with normal flora can be avoided by the use of invasive methods of specimen collection. Patients and methods. We examined specimens obtained at bronchoscopy (bronchial washings, bronchoalveolar lavage - BAL, transbronchial specimens), lung biopsy specimens and thoracentesis fluid. All specimens were stained with Gram's and acridine orange stains, plated on various agar plates and incubated under aerobic and anaerobic conditions. Bronchial washings and BAL specimens were evaluated quantitatively. Antimicrobial susceptibility of all isolated bacterial strains was determined by the disk diffusion test. Results. Thoracentesis fluid specimens constituted 349 (66,7%) of all specimens obtained, yet the growth of microorganisms was detected in only 26 (7.5%) of them. A much higher percentage of bacterilogically positive results was yielded by other types of specimens: BAL 80%, bronchial washings 54%, mini BAL 52%, and biopsy specimens 40%. Gram-positive cocci predominated in all types of specimens. From 114 specimens, 120 bacterial starins and 33 fungal isolates were recovered. Potentially pathogenic bacteria included: Staphylococcus aureus, Streptococcus pneumoniae, Gemella morbillorum, enterobacteria, Pseudomonas aerugionosa, nonfermentative bacilli, Steptococcus agalactiae, Enterococcus sp., Haemophilus infuenzae, neiseria meningitidis, Streptococcus intermedius, bacteroides sp. Summary     Izhodišča: Z uporabo invazivnih metod odvzema vzorce iz spodnjih dihalnih poti se lahko izognemo kontaminiranju vzorcev z normalno floro in kolonizatorji zgornjih dihalnih poti. Bolniki in metode. Pregledali smo vzorce 86 bolnikov, odvzete pri bronhoskopiji (izpirek bronhija, bronhoalveolarni izpirek, bronhoskopska pljučna biopsija), plevralne izlive ter material, odvzet s pljučno igelno aspiracijo. Vsi materiali so bili kultivirani v aerobnih in anaerobnih pogojih. Pred bakteriološko obdelavo pa smo iz vseh kužnin naredili razmaze, obravnave po Gramu in z barvilom akridin oranž. Pred zasajanjem na gojišča za osamitev mikroorganizmov (bakterije in glive) smo izpirke bronhijev in bronhoalveolarne izpirke kvantitativno obdelali. Pri vseh poraslih bakterijskih sevih smo napravili difuzijski antibiogram. Rezultati: Najpogostejši vzorci so bili plevralni izlivi, ki predstavljajo 349 (66,7%) vseh invazivno odvzetih vzorcev. Rast mikroorganizmov pa je bila prisotna le pri 26 (7,5%) plevralnih izlivih, pri 4 (80%) bronhoalveolarnih izpirkih, pri 62 (54%) izpirkih bronhijev in pri 20 (40%) materialih iz pljučne periferije. V vseh pregledanih kužninah prevladujejo po Gramu pozitivni koki. Iz 114 vzorcev smo osamili 153 sevov mikroorganizmov. Bilo je120 sevov bakterij in 33 sevov glivic. Potencialno patogene bakterije so zastopane v 60 sevih, glivice pa v 20 sevih. med potencialno patogenimi smo osamili.
