Abstract     During the past 14 years 1844 patients (20.2%) with ejection fraction lower than 30% underwent myocardial revascularization. There were 1648 male and 196 female patients with average age of 54.5 years. The majority of the patients (55%) had previous myocardial infarction on anterior wall, 27% of them on the posteroinferor, and 12% on both walls. Postoperative mortality in the whole group of 1844 pts was 3.6% and for the group of patients with ejection fraction lower then 20% it was 7.5%. The majority of them died due to cardiac causes. From the group of 290 analysed coronary patients, there were 56 pts (19%) with poor left ventricular function - ejection fraction of 30% and lowr. We have compared results at 9 years and have found statistically significant difference insurvival between the group of the patient with ejection fraction higher than 30% (89.45% +- 2.3 at 9 years) and those with failing left ventricle (66.31% +- 6.6 at 9 years). Our operative risk in patients with poor left ventricular function - ejection fraction <= 30% is 3.6% and is low, compared with the data of the literature. The long term result shows survival of 66% at 9 years. We conclude that this severe group of coronary patients can derive benefit from myocardial revascularization.
