Abstract     During the development of proliferative diabetic retinophaty virteous hemorhages and proliferative membranes start to prevent the formation of the image on the retine. Photocoagulation, which impedes the neovascularization process, can be done. Further developement of the disease leads to traction detachment and blindness. The goal of surgical treatment of diabetic retinopathy is: to clean the optical axis and allow the formation of retinal image, to remove the vitreous body as a scaffold for membrane growth, to remove the neovascular membranes, the source of bleeding and traction, to reattach the retina, to prevent further neovascularisation by foto and cryocoagulation. The success of this treatment depends on several factors: previous panretinal fotocoagulation, appropriate timing of vitrectomy, surgical technique, optimal diabetes treatment, general medical condition of the pacient, other complications of diabetes. The prevention of blindness of diabetic patients starts at the level of general practicioner and diabetologist and is carried on at the level of general ophtalmologist, laserist and vitreoretinal surgeon.The awareness of surgical indications and the possibilty of not delayed surgical treatment substantially improve the visual prognosis for the pacients.The role of national health-service and national eye-care service is of paramount importance.
