Abstract     The aim of this study was to establish the connection between preoperative serum CA 125 values and the parameters of serous tumors aggressiveness, in the author's opinion represented by DNA aneuploidy and high S-phase fraction (SPF). The author's next task was to establish which factor best predicts the survival of patients with ovarian carcinoma. Methods: In this retrospective study 28 patients with cystadenoma, 44 patients with borderline carcinoma and 63 patients with serous carcinoma were included. In all patients the preoperative serum CA I 25 level was obtained. CA 125 immunohistochemistiy was performed on the most representative tissue block section from which also suspension of the nuclei was prepared by the Hedley method. DNA ploidy and %S were determined by means of flow cytometry. Statistical methods: Mann-Whitney U-test, Spearman's correlation, standard and stepwise multiple regression, logistic and Cox regression, Kaplan-Meier survival analysis. Results: The high serum CA125 values are significantly connected to SPF (p = 0,0045) and to DNA aneuploidy (p = 0,017), and both parameters are on the other hand significantly connected to the decreased CA125 tissue expression. The correlation between serum and tissue CA125 expression was negative and non-significant (p = 0,063). DNA aneuploidy is significantly connected only to tumor grade (p = 2,1 E-09) and to high SPF (p = 0,0062). When serum CA125 levels are below 65,0 U/ml, the risk of DNA aneuploidy is approximately 15%. With the increase of seium CA125 levels the risk of DNA aneuploidy also rises and exceeds 70% at CA125 values higher than 500,0 U/ml. Among prognostic factors DNA aneuploidy is the best predictor of the survival of patients with ovarian serous carcinoma (p = 0,0037). (trunc. at 2000 chars)
