Abstract     A retrospective survival analysis of patients with stomach cancer, reported to the Cancer Registry of Slovenia in the period 1988-1990, was aimed to assess the possible differences in survival of radically resected patients treated in three groups of hospitals, and some prognostic factors of their survival. The group 1 included Department of Gastroenterologic Surgery and Department of Thoracic Surgery, UMC Ljubljana, the other two groups were defined according to the average number of radical resections performed yearly (under or over 15). The total of 1.380 patients with stomach cancer with exclusion of lymphomas, sarcomas and carcinoids were included in the analysis. The survival was estimated by the method of relative survival rate and the prognostic significance of several factors was evaluated with a proportional hazards model, adapted to the use of the relative survival. Of all patients included, 38.0% were diagnosed with metastatic disease. Out of 68.3% of patients treated surgically, only 46.5% were radically resected; 45.4% of patients were treated with supportive care only. The relative five-year survival rate for all stomach cancer patients was 16.4% (95% confidence interval 14.1-18.7), while for radically resected patients it was 43.1% (37.6-48.6); it was significantly lower in patients with regional disease, but on the other hand, no differences in survival by hospital groups were noted. The multivariate regression model with the best fit to our data included the following variables: age, stage of disease and year after diagnosis. The variable hospital group did not change the fit of the model to the data significantly. On the basis of the relative risk estimates we concluded, that stage of the disease was the most important prognostic factor in radically resected patients with stomach cancer in Slovenia.
