Abstract     A 46-year-old man was admitted to the hospital because of severe pain in his back and sudden weakness in his low extremities. Clinically suspicion of aortic dissection has not been confirmed neither by transesophageal echocardiography nor by aortography. A retroperitoneal haematoma was suspected on the results of abdominal ultrasound and some laboratory parameters. The CT confirmed aortic dissection type B and solid tumour in left suprarenal gland. We decided for medical treatment in the absence of complications of distal aortic dissection. The patient remained hypertensive with paroxysmal hypertensive crises despite of medical treatment. Phaeochromocytoma was suspected and the diagnosis was made by high urinary metanephrine levels.J-131 MIBG imaging confirmed the existence of phaeochromocytoma in the left suprarenal gland. Phaeochromocytoma was confirmed histologically following an uncomplicated left adrenalectomy. Postoperative metanephrine levels were normalised and hypertension was treated with much lower doses of antihypertensive drugs. Postoperative control CT did not confirm any progression of aortic dissection. Further course was uneventful.
