Abstract     We report a case of a 45-year old patient who suddenly collapsed on the street. Beside his head there was a puddle of blood. When emergency team arrived, he was unconscious, not breathing, and had no pulses. In 30 minutes they achieved return of spontaneous circulation. At the admission to the Intensive care unit (ICU) the patient was in shock, unconscious, and artificially ventilated. Gastroscopy showed a clot of blood on the dorsal wall of the stomach, which could have not been removed. The loss of blood was contributed to the bleeding from the upper gastrointestinal tract. The patient died on the day 4 because of massive bleeding which occured during the aspiration of the respiratory tract. The autopsy revealed cavernous tuberculosis of the upper pulmonary lobe with massive hemorrhage.
