Abstract     The most important blood function is oxygen transport, need by the tissues for aerobic metabolism. Other important functions includes immunological and coagulation function. Advanced technology enable us to separate different blood components, so we can provide specific component therapy. Which component we sill use depends on clinical and haemodynamic status of the patient. We also know that blood transfusion is introduction of the foreign material, which can cause immunological response of the organism with serious anaphylactic reaction, haemodynamic and respiratory consequences. Other serious concern is viral infection (hepatitis, AIDS). In emergency transfusion we usually use full blood, concentrated erythrocytes, fresh frozen plasma and thrombocyte plasma, depending on magnitude of blood loss. In fluid resuscitation, it is important to replace lost ciruclating volume and oxygen carying capacity. Small volume can be replaced only with crystalloid and colloid solutions while lost erythrocytes are replaced by the organism. Role of the nurse in the emergency transfusion is to: 1. acquire a blood sample 2. to prepare documentation 3. to prepare for blood type determination 4. to establish i.v. access and give the transfusion 5. to prepare blood warmers and infusion pumps 6. to organise blood storage.
