Abstract     Antineutrophil cytoptasmic antibodies (ANCA) are a useful marker in the diagnosis of pauci-immune necrotizing crescentic glomerulonephritis with or without systemic vasculitis. These autoantibodies are directed against various cytoplasmic antigens of neutrophil granulocytes and monocytes, the most frequent being proteinase 3 and myeloperoxidase. Two staining pattems are recognized by indirect immunofluorescence: cytoplasmic and perinuclear. ELISA enables the determination antibodies' antigen specificity. Antibodies with specificity for proteinase 3 are highly sensitive for Wegener's granulomatosis and anti-myeloperoxidase antibodies are characteristically found in patients with microscopic polyaniitis and pauci-immune creseentic glomerulonephritis. The term ANCA associated diseases has generally been accepted. However, ANCA can occasionally be positive in various other diseases. mostly in low values. with an atypical immunofluoreseence pattern and specificity for iiiyeloperoxidase or unknown antigen.
