Abstract     Botulinum treatment has proved a promising treatment for many dystolic and spastic disorders. In blepharospasm and torticolis improvement can be dramatic, however it is usually more subtle in limb dystonia and spasticity. Apart form correction of posture and pain relief, functional testing is an important part of pre - and post - treatment assessment. We report problems and pitfalls of assessment scales and scoring as used in a double - blind cross over study. Included are 10 patients with upper limb motor disorders. The following assessment scales were used: a modified Frenchay Arm Test (mFAT), selfassessment of functional improvement and pain relief on a 0 - 5 scale, patient's guess on the substance used,Brin's Global clinical Rating Scale, and Clinican's Global Assessment. While there was a (non)significant improvement on a mFAT for the botulinum session, a more evident difference in favour of treatment was found on "global" scales, particulary the 0 - 5 self - assessment scale and the patient's gues. We conclude that MFAT is unsufficiently sensitive and not superior to simpler, semi - qualitative and global scales and that measurements of focal disability do not sufficiently clarify functional changes after treatment with botulinum toxin. Therefore the measurement of handicap can be more appropriate.
