Abstract     Renal osteodystrophy, one of the most frequent complications in chronic renal failure, is characterized by an entire spectrum of histologic disorders: from high to extremely low bone activity turnover. There are several causes for one or another form of the disease. The aim of this study was to establish the correlations between intact parathormone (iPTH) concentration in serum bone densitometry and histomorphometric bone parameters. We determined serum iPTH concentrations, bone mineral density with densitometry and performed bone histomorphometry - static and dynamic(determination of bone volume, osteoid volume, peritrabecular fibrosis, osteoid thickness, osteoblast and osteoclast interface, - double tetracycline trace and mineral appositional rate). We also calculated the correlation between iPTH concentrations, bone mineral density and individual bone histomorphometric parameters. Bone densitometry and determination of iPTH concentrations are methods applied in diagnosing renal osteodystrophy, each individual method, however, is not sufficient for making the diagnosis. The dominant etiopathogenetic type of renal osteodystrophy can not be determined with certainty merely on the basis of one method, and this is esential to the choice of therapy. This study proves that without additional parameters not even the combination of these methods can increase their diagnostic potential significantly. With additional parameters, i.e. X-ray, AP, P and patient age, we succeeded in classifying 73.3% of patients correctly according to diagnosis. Bone histomorphometry was used as areference method for making the definite diagnosis of the type of renal osteodystrophy.
