Abstract     Intravascular stenting with various mechanical devices has improved minimally invasive therapy of obstructive peripheral and coronary artery disease. Stents provide excellent scaffolding, predictable immediate success with the creation of a large, dissection-free luminal cross-section and improved long-term results, when compared to ballon angiopasty. Despite these improvements restenosis still occurs particularly in smaller vessels and in longer lesions regardless of the specific anatomic site. Early experiences with politetrafluorethylene covered stent graft showed that stent grafts in occlusive disease may limit the ingrowth of intimal hyperplasia along the lenght of the treated segment with improving patency as compared to conventional angioplasty and stenting. Endovascular grafts are used as well to bridge the vascular segment with aneurysm, penetrating trauma, pseudoaneurysm and A-V fistulas. Future developments in stents and stent grafts include the delivery of pharmacological agents, radiation and gene therapy. Research is already starting to replace synthetic materials with biological and gene enegineering processes designed to stimulate the body to build in place its own blood vessel.
Summary     Vstavljanje znotrajžilnih opornic različnih tipov je izboljšalo minimalno invazivno zdravljenje zapor perifernih in koronarnih arterij. Znotrajžilne opornice (ang. stenti) delujejo kot ogrodje, ki po balonskem širjenju ohranja žilno svetlino razpeto, preprečuje prosto gibanje raztrganih delov žilne stene in zagotavlja boljši dolgoročni rezultat zdravljenja kot sama balonska angioplastika. Kljub omenjenim prednostim še vedno prihaja do restenoz zlasti v drobnejših žilah in pri daljših žilnih zožitvah ne glede na antomsko mesto. Zgodnje izkušnje z opornicami, pokritimi s politetrafluoretilenom, ki jih imenujemo znotrajžilne proteze, kažejo, da takšne žilne opornice zmanjšujejo intimalno hiperplazijo vzdolž zdravljenega segmenta žile. To izboljšuje dolgoročno uspešnost zdravljenja v primerjavi s klasično angioplastiko ali vstavljanjem običajnih žilnih opornic. Znotrajžilne proteze uporabljamo tudi pri premoščanju žilnih anevrizem, penetratnih poškodb žilja, psevdoanevrizem in arterio-venskih fistul. Razvijajo nove tipe žilnih opornic in znotrajžilnih protez, ki bodo sposobne dostavljati farmakološke učinkovine, raziskave pa se povezujejo tudi z dosežki genskega zdravljenja in radioterapije. V perspektivi bodo sintetične materiale nadomestili biološki proizvodi genskega inženiringa, ki bodo vzpodbudili telo, da bo na njihovem mestu razvilo novo krvno žilo.
