Abstract     Urinary tract infections are common, ranking in frequency of occurrence with respiratory and gastrointestinal infections. The reasons are to be found in a complex interaction between microbial virulence factors and host resistance mechanisms. Pyelonephritis means inflammation of the kidney and its pelvis, most common bacterial. The most important causes of urinary tract infections are Enterobacteriacecae (among them Escherichia coli) and Enterococcus faecalis, accounting for more than 95 % of all urinary tract infections in all population groups. Acute pyelonephritis is more common ascending infection resulting from ascension of organisms from the urethra into the bladder and then by the ureter to kidney, less frequently it is hematogenous infection. The clinical manifestations of acute pyelonephritis are fever, chills, back and loin pain, often with colicky abdominal pain, nausea and/or vomiting. The onset is commonly acute and may or may not be preceded by bladder symptoms.
