Abstract     Every day the physician comes across the problem of cardiac irregularity, at the hospital and the OPD as well as in the field. The diagnostic approach to the patient with arrhythmia, giving priority to the electrocardiogram owing to the usually urgent condition, differs from the classic approach of internal medicine. Usually the experienced physician can identify most arrhythmias very quickly by means of simple procedures, but sometimes demanding invasive diagnostic procedures are required for their accurate determination. The first and most important question raised in cases of arrhythmia is: "Is the patient hemodynamically at risk and if so, is arrhythmia the cause?" The answer should dictate all further measures. Accurate analysis of the electrocardiagram, the interdependence of its individual segments, the possibilities of AV dissociation as well as the width and morphology of the QRS complex, with special stress on the discernment of P waves, most often lead us to the final answer regarding the type of cardiac irregularity. The specific history is followed by the usual physical examination with observation of the most expressive elements of arrhythmia such as the intensity of the first heart sound, the split second heart sound, jugular pulse and the execution of certain vagal maneuvers. In conclusion it should be stressed that a final etiologic diagnosis requires additional investigations, known in cardiology and internal medicine respectively, since arrhythmias are mostly not diseases by themselves but rather symptoms of heart diseases as well as of other organic or systemic diseases.
