Abstract     Apart from acute complications of diabetes (diabetic ketoacidosis, aketotic hyperosmolar syndrome and hypoglycemia), urgent endocrinological states are rare. Among them is the Addisonian crisis (acute adrenal cortical insufficiency, which may develop in chronic adrenal insufficiency in cases of insufficient treatment or as the initial manifestation of adrenal insufficiency. The Addisonian crisis occurs because of a cortisol deficit. Owing to abdominal pain, vomiting and diarrhea, it could be mistaken for an acute abdomen. Treatment (saline infusions, cortisol substitution, treatment of infection or other releasing cause respectively) must be introduced even before the results of hormone investigations confirm the diagnosis of the disease. The description of a case is presented.
