Abstract | Multiple organ dysfunction syndrome (MODS) and multiple organ failure syndrome (MOFS) are common causes of mortality in critically ill children who survived the initial phase of treatment after a severe injury (soft tissue injury, major burns, multiple fractures), intraoperative stress or sepsis. MODS and MOFS develop after a certain latent period in the context of a host reaction, from an adeqate adaption response to the destruction of multiple organ systems and death. The authors describe the current pathophysiologic mechanisms in MODS and MOFS, focussing on the role of the multifunctional capillary endothelium, peripherial vascular insufficiency with maldistribution of flow and oxygen. They describe the role of polymorphonuclear leukocytes (PMNL), inflammation mediators, gut mucosa, coagulation and fibrinolysis as well as complement cascade in MODS and MOFS development. Wilkinsons clinical diagnostic criteria, some new diagnostic methods and principles of treatment in children with MODS and MOFS are also presented. | |
