Abstract     In acute stage after severe head trauma in cases with intracranial haematomas the neurosurgical procedures are essential for prevention of irreversible brain damage and brain death. Clinical and diagnostic work-up must be fast and effective. The secondary brain damage which lead to the cytotoxic brain edema should be prevented. The consequences of severe head and brain trauma, morphological changes and clinical considerations are described.
