Abstract | The prevalence and the mortality of asthma increase. Most asthma deaths are neither sudden nor unexplained, rather they are due to under-recognition, poor education and under-treatment. Acute severe asthma is a medical emergency. The initial management comprises reassurance, oxygen, beta2-adrenergic agonists and steroids. The use of theophylline is controversial. Intubation and ventilation is associated with a danger of barotrauma and hypotension. Elective hypoventilation with permisive hypercapnia and low peak inflation pressure is the ventilatory technique of choice. | |
