Abstract     Measurement of only myoglobin and the percentage of CK-MB (activity) are sufficient in about 95% of all requests for laboratory diagnosis, confirmation or exclusion of myocardial injury. Most receantly cardiac troponin I (cTnl) was proposed to be more reliable marker for myocardial injury than CK, CK-MB or LDH. We evaluated several pathologic conditions where serum cTnl might be superior to the percentage of CK-MB. We confirmed that cTnl might be a useful marker for the late detection of myocardial injury, is more specific in case of injury of skeletal muscle enriched with CK-MB, is more specific in case of the presence of CK macro type II or I and is useful when myocardial and skeletal muscle injury are coincided. However unexplained elevated levels of cTnl were frequently detected in patients with chronic renal diseases.
