Abstract     Vascular changes have a decisive influence on the shortening of renal allograft survival, but they have not yet been systematically studied in all vascular segments. Our hypothesis was that the entire vascular system of the allograft may be a target of the recipient's immune attack, although we assumed that differences exist between segments, as a consequence of the antigenic peculiarities of the vascular endothelium. The study was performed on surgically removed kidney allografts with hyperacute rejection (n =1), acute rejection (n = 8), chronic rejection (n = 21) and nonrejection thrombosis (n = 8). Comparative groups of 31 normal kidneys and 20 kidneys of patients with benign arterial hypertension taken during autopsy were also included. Traditional light microscopic, and on selected specimens, also electron microscopic studies were used to evaluate histomorphologic changes in all segments of the intrarenal and extrarenal vessels. Using specifc antibodies, we also imunohistochemically assessed endothelial cells (CD31), smooth muscle cells (smooth muscle actin-alpha), T lymphocytes (CD3), B lymphocytes, plasma cells (CD79a), and macrophages (CD68) in the vessel walls, as well as extracellular components using antibodies directed against collagens I, III and IV, laminin, fibronectin, and elastin. Expression of adhesion molecules ICAM- l, VCAM-1 and E-selectin, and markers of immune activation HLA-DR and the interleukin-2 receptor (IL-2R) were also evaluated. Deposition of immunoglobulins, complement components and fibrin/fbrinogen were assessed by the direct immunofluorescence microscopic technique. The intensity of histomorphologic changes, immunohistochemical characteristics and immune deposits was determined semiquantitatively as mild (1 +), moderate (2 +) or considerable (3 +).
