Abstract     All of deep structures of the upper extremity and the skin distal to the middle of the upper arm are rended insensitive by blocking the nerves making up the brachial plexus. The nerves of the plexus may be blocked anywhere along their course from intervertebral foramina and entrance into the sheath between the interscaleni muscles until they terminate in the specific nerves in the hand. Techniques for blocking of the plexus involve infiltration at one of five anatomic sites: that is -paravertebral(interscalene), -supraclavicular, -ifraclavicular, -axillary, and -by blocking the specific terminal nerves. Infraclavicular vertical approach is a new method for blockade of brachial plexus(1). Compared to other techniques, this is a highly successful method. It is easy to describe and teach, it has clear surface landmarks, it has low risk and high acceptance by patient and anesthesiologist. The patient tolerates the upper arm tourniquet. The block has very rapid onset of complete anesthesia and long-lasting postoperative analgesia.
