Abstract     The author defines successful and unsuccessful root canal fillings, lists the objective and subjective causes of failure, and suggests ways how problems can be overcome. The objective causes of failure include unfavourable anatomical conditions, the existence of a "step" in the apical area of the root canal resulting from inappropriate instrumentation, bacterial resistance, an awkward position of the tooth or its remnant, and an unfavourable psycho-physical status of the patient. The subjective causes of unsuccessful endodontic treatment comprise a false diagnosis, unclean work, failure to obtain a radiograph, inadequate access to the pulp chamber and hasty work.
