Abstract     The paper describes the clinical management of a female patient with bilateral cleft lip and palate from birth until the age of 39 years. The surgical procedures, orthodontic and periodontal treatment are described. Special emphasis is placed on the aesthetic and functional prosthodontic rehabilitation, accomplished 17 years ago when the patient was 22 years old. A fixed construction connects the teeth of the transcanine region (veneer crowns) with a bar in the arch, bridging the gap of the missing incisors. Both canines and one lateral incisor are covered by conus crowns. The cast partial denture is constructed on milled conus crowns and a milled bar, retained by friction and a system of clasps. After 17 years of use, the construction is still firm and aesthetically and functionally appropriate. This is due to optimal planning and technical execution as well as to proper oral hygiene, carefully maintained by the patient.
