Abstract     Objective.The combination of bone marrow transplant together with solid organ transplantation has been suggested to augment chimerism and facilitate the developmnet of donor specific tolerance. The grade of graft-versus-host disease is supposed to depend on the dinamic process of immune response activation within the recipient. By the use of different immunosuppressive substances it is possible to interfere in the two-way dynamic process between the recipient of the solid organ and transplanted organ. Methods. 35 white-large type piglets underwent isolated orthotopic small intestinal transplantation. All experimental animals were divided into three groups based on the described protocol. Vertebral bodies were used as a favorable alternative to iliac crest as a source of bone marrow cells to provide an appropriate number of the same cell type per kilogram of recipient body weight. Liquid storage of isolated bone marrow cells at +4 C provides an appropriate quality of the same cell type for transplantation. We compared the histologic grades of skin and intestinal GVHD in the recipient of combined bone marrow and orthotopic small intestinal transplant and orthotopic small bowel, only. The next clinical parameters were used: survival in days postoperative, cause of death, recipient preoperative and final body weight, the presence of diarrhea, the number of infections per animal and intraoperative findings in second look surgery. We used the next laboratory parameters: the level of FK 506 in recipient sera, electrolytes and blood cell count, hemocultures, the samples of isolated bone marrow cells for aerobic and anaerobic culture, vitality and quality studies. Results. We did not observe a statistically important number of surgical complications in the pig model of orthotopic small intestinal transplantation. (Abstract truncated at 2000 characters).
