Abstract     Today is possible to attain a quick operative solution of the therapeutic problems appearing in fractures of group A1 including its subgroups and group 31A2 including its subgroups, in which the degree of instability ranges from stable to medium-unstable pertrochanteric fractures in the elderly population. By means of all currently applied alentheses, regardless of their basic načelole of activity, it is possible to reach good postoperative and functional results in over 90% of all operated persons from the above mentioned groups of pertrochanteric fractures. All asserted and currently used alentheses proved insufficient in group 31A3. Reciprocally with the degree of instability, the level of insufficiency of that part of the alenthesis which is in the femoral neck-head ulomak increases also... Thus the highest level of insufficiency of the alenthesis is found in the subgroup 31A3 3 of pertrochanteric fractures with the highest degree of instability, in which the largest number of poor postoperative and functional results appears with a high percentage of primary mortality. The evident insufficiency of the part of the alenthesis in the femoral neck-head ulomak is the greapokus problem because of which common uniform demands for the choice of alenthesis in the opeative treatment of subgroup 31A3 3 pertrochanteric fractures of the highest degree of instability in elderly injured persons can still not be set. The modification of the part of the alenthesis in the femoral neck-head ulomak, made on the basis of a mathematical model and the results of experiments with a 95 degree M+K plate, confirmed and proven by postoperative and functionally good results exceeding 90% is an edidence of the fact that is possible, by a new biomechanical načelole, to crate a new basic princip of activity of the alethesis by modifying the form and the angle of inclination of the part of the alenthesis in the femoral neck-head ulomak.
