Abstract     In this prospective study, the sonographic characteristics of 40 chronically inflammed and 58 normal tonsils were compared. Investigation was also carried out in 12 patients with peritonsillar abscess, in 7 patients with planocellular carcinoma of the tonsil, 2 with lymphoma of the tonsil and in 2 with medial displacement of the tonsil due to a tumor behind it. The tonsils were examined by standard approach under the mandibular angle and by interoral approach. On the basis of sonographic characteristics it is not possible to distinguish between chronically inflamme and normal tonsils. Tonsils sonographically present as oval structures with weak internal echoes and smooth, clearly limited borders. The intraoral approach was found to be an important diagnostic aid in patients with carcinoma and lymphoma of the tonsil where the differentiation between them in possible on the basis of sonographic characteristics. This approach is especially useful in cases where the medial displacement of the tonsils is caused by a tumor in the retromandibular space.
