Abstract     Penetrating keratoplasty "a chaud" is indicated as a surgical procedure of corneal ulcer with or without perforation. Methods: Two cases of extremely severe spontaneous corneal ulcer perforation are presented on the video. Results: The first patient was referred to our Department with clinical picture of perforated corneal ulcer with the signs of anterior endophthalmitis and secondary glaucoma. The second patient was operated because of large corneal perforation on distrofic cornea partialy obliterated with dislocated cataract. Penetrating keratoplasty with rotor trephine 8,0 mm of diameter was performed in both cases because of the extreme extension of the lesions. During dissection of severe synechiae between very hyperemic iris and cornea up to the anterior chamber angle, came to lasting bleeding in both cases. Bypolar cautery and viscoelastic tamponade were used to stop bleeding. In both cases open sky vitrectomy was performed. We did not decided for IOL implantation. Conclusions: Despite marked disorganization of the anterior chamber globe could be saved isn both cases. Three months after operation, visual acuity was light perception because of severe intraoperative vitreous haemorrhages. Intraocular pressure was normal.
