Abstract     Background: Triple procedure includes penetrating keratoplasty, extracapsular cataract extraction and lens implantation. Methods: On this video the triple procedure with small pupill following micotic perforated corneal ulcer covered with conjunctival flap with severe signs of anterior uveitis was presented. Results: Trephination of the cornea was done with 7,2 mm rotor trephine. Synechiolysis with viscoelastic was performed. Epipullary membrane was removed with scissors. The pupil was dilatated with flexible iris retractors. Extracapsular "open-sky" cataract extraction was performed in the usual way. The visibility during the surgery was better and the procedure saftier. Conclusion: The use of flexible iris retractors in small pupil "open-sky" cataract extraction in triple procedure improves the visibilty during surgery, facilitates the expression of the nucleous and the aspiration of the cortex and makes the procedure easier.
