Abstract     Purpose: To determine the efficacy of trabeculectomy with mitomycin C (MMC) in neovascular glaucoma and to compare our results with those in the literature. Methods: Trabeculectomy with intraoperative MMC application (0.4 mg/ml, 4 minutes) was performed in 22 patients (23 eyes) with neovascular glaucoma (due to diabetic retinopathy in 14 and retinal vein occlusion in 8 patients). The definition of success was IOP of 22 mmHg or less with or without medication. Results: The mean follow up was 8.9 months (range,1- 24 months). The mean preoperative IOP was 43.3 +- 11.8 mmHg (SD) , the mean postoperative IOP 20.8 +- 12.0 mmHg (SD). The mean IOP reduction was 52%. Sixteen patients (17 eyes) had IOP equal or less than 22 mmHg and in 19 out of 22 patients the IOP reduction was greater than 20%. The operation was unsuccessful in 6 patients (27%). The visual acuity deteriorated in 7 patients, in two despite controlled IOP. Conclusion: Trabeculectomy with MMC in our patients with neovascular glaucoma proved to be as successful in lowering IOP and preserving visual function as transscleral laser photocoagulation or Molteno drainage device implantation although the comparison with other authors remains difficult due to different criteria of success, heterogeneity of glaucoma subtypes, different MMC concentration and exposure time and variability in follow-up.
