Abstract     Purpose: To determine the efficacy and safety of latanoprost treatment in glaucoma patients in whom maximal medical therapy has been insufficient to control intraocular pressure (IOP) in order to stop the progression of glaucomatous neuropathy. Methods: Following initial ophthalmological examination latanoprost was added to maximal tolerated therapy once daily in the evening to 20 patients ( 16 primary open-angle glaucoma, 2 juvenile open-angle glaucoma, 2 posttraumatic glaucoma). The mean follow-up was 10 months. Results: Latanoprost significantly (P < 0.05) reduced IOP from 25.0+- 4.9 mmHg before latanoprost treatment to 20.8+- 3.5 mmHg (17% reduction) at the last control visit. There was no long-term drift of IOP. Latanoprost caused increase in iris pigmentation in 3 (15%) of the 20 patients, all of whom had multicoloured irides at baseline. In 2 patients with unilateral latanoprost therapy longer and darker eyelashes were noted. None of the patients had to discontinue latanoprost treatment due to local or systemic side-effects.
