Abstract     Lower motor neuron abnormalities after stroke have been studied with different methods by many authors. In this study we assessed neuromuscular transmission in weak muscles of patients after acute ischaemic stroke. We used single fiber electromyography (SFEMG) with axonal microstimulation, which allows detailed studies of neuromuscular transmission at individual motor end plates. The results show that a majority of the motor end plates in our stroke patients were normal. Never the less the jitter on the paretic side was slightly larger, the difference respective to the unaffected side being statistically significant. The number of motor end plates with jitter larger than 40 us was 6,9 % on the paretic side, compared to only 0,5% on the unaffected side. The study was designed to assess the original motor end plates before collateral sprouting and reinervation could have taken place. The results are believed to support the hypothesis of transsynaptic degeneration.
