Abstract | Exulceratio simplex Dieulafoy is a rare cause of serious bleeding in the upper gastrointestinal tract. This vascular malformation was described by the French surgeon Georges Dieulafoy already in 1896 and is named after him. Massive hemorrhage occurs as a result of ruptured unusually large submucosal artery, that travels a tortuous course through the submucosa. There is no ulcer around the lesion, which allows the endoscopist to distinguish the Dieulafoy's lesion from a vissible vessel within a gastric ulcer. The site of bleeding is usually located 6 cm below the gastroesophageal junction, predominantly on the lesser curvature. The clinical picture of this lesion is intermittent massive bleeding in the upper digestive tract, often without any bleeding source revealed at emergency endoscopy. Interventional endoscopy has significantly influenced surgical management and reduced mortality. | |
