Abstract     Background. Chronical periapical parodontitis (CPP) is the result of a pathological immune response to the continous stimuli from the inflammated dental pulp. CPP represents chronical focus of the inflammation.The purpose of our study was to investigate CPP in sense off the immune response by flow cytometry (FC). Methods. 47 CPP samples were obtained after surgical removal-apicoectomy. Each CPP sample was examined by histological, bacteriological and flow cytometrical analysis of lymphocytes infiltrating CPP samples. We used pathohistologic and microbiologic findings and compare them to the results of immunological analysis. Results. By FC we found a significant increase in proportions of Tlymphocytes expressing interleukin-2 receptors and ICAM 1 compared to peripheral blood lymphocytes. A decreased expression in proportions of cytotoxic Tlymphocytes (CD8) expressing interleukin-2 receptors and HLA DR molecules were found in CPP samples predominantly colonized by Streptococcus spp., compared with lesion predominantly colonized by strict anaerobes. Proportions of T helper cells that produce interferon-gama (INF-gama) was higher in CPP samples predominantly colonized by anaerobic bacteria. There were no differences in IL-4 expression by T cell in both groups (anaerobic and streptococcal). Among anaerobic CPP samples differences in proportion of T cells that express IL-2 receptors expression was also found between samples colonized by P. acnes and Bacteroides spp. Conclusions. Oral streptococci cause relatively limited tissue destruction and induce Th2 type of immune response accompained by non-cytotoxic inflammatory reaction. On the contrary, anaerobic bacteria induce Th 1 type of immune response that cause severe inflammatory reaction (type 4 of hypersensitivity) that damage the tissue by the action of cytotoxic T cell activation.
Summary     Izhodišča. Kronični periapikalni parodontitis (KPP) je rezultat patološkega imunskega odziva na kontinuirano draženje vnetno spremenjene zobne pulpe in predstavja kronično vnetno žarišče. Namen naše naloge je bil preučiti imunološke značilnosti KPP z uporabo pretočne citometrije (PC). Metode. 47 vzorcev tkiva KPP smo odvzeli pri kirurškem posegu - apikotomiji. Vsak vzorec KPP je bil pregledan s standardno patohistološko tehniko (HE), bakteriološko preiskan z metodo PC pa smo ovrednotili limfocitne podvrste in njihove funkcionalne lastnosti. Rezultati. Patohistološke in mikrobiološke analize smo vzporejali z izsledki imunoloških raziskav. S PCsmo ugotovili statistično značilno večje deleže limfocitov T, ki so izražali receptorje za interlevkin 2 (IL-2) in adhezijske molekule ICAM 1 v vzorcih KPP v primerjavi s periferno krvjo. V vzorcih KPP, kjer so prevladovali ustni streptokoki, smo ugotovili manjši delež citotoksičnih limfocitov T (CDS), ki so izražali receptorje za IL-2 in molekule HLA DR v primerjavi z anaerobno skupino KPP. Večji delež limfocitov T-celic pomagalk je v anaerobni skupini vzorcev KPP tvorilo interferon gama (IFNgama) v primerjavi s streptokokno skupino KPP. Med anaerobno in streptokokno skupino vzorcev KPP nismo izmerili razlik glede izražanja IL-4 na limfocitih T celicah pomagalkah. Znotraj anaerobne skupine vzorcev KPP smo ugotovili razlike v deležu limfocitov T, ki so izražali receptorje za IL-2 med vzorci KPP, kjer je bil povzročitelj P. Acnes, in vzorci, kjer je bil povzročitelj Bacteroides spp. Zaključki. Ustni streptokoki razmeroma malo okvarjajo tkiva in spodbudijo Th2 tip imunskega odziva, ki vodi v blago vnetno reakcijo. Anaerobne bakterije spodbudijo Th1 tip imunskega odziva, ki povzroča močno tkivno vnetje. Vnetje je pred vsem citotoksičnega tipa in ga lahko uvrstimo v tip 4 preobčutljivostne reakcije.
