Abstract     Background: The prevalence of coeliac disease is constantly increasing. The disease is known to be more common in certain groups. The prevalence of coeliac disease among family members is estimated to be between 2% and 12%. In order to assess the prevalence of the disease among family members of our patients we screened 80 first-degree relatives of 31 coeliac disease patients. Patients and methods: We analysed antigliadin (AGA) and antiendomysium (EMA) antibodies in all family members. Small bowel biopsy was recommended in family members with positive serology. Biopsy specimens were examined histologicaly. The prevalence of the disease was calculated as the number of coeliac disease patients among 100 first-degree relatives. Results: Three family members (3.75%) were both AGA IgG and EMA positive, one (1.25%) was only EMA positive, and ten (12.5%) were only AGA IgG positive. Small bowel biopsy was performed in five family members, and the final diagnosis has been confirmed in four. The prevalence of coeliac disease among family members can therefore be estimated at 5%. Conclusions: The prevalence of coeliac disease among first-degree relatives is high, and the majority do not have a typical form of the disease. Without an active search, the disease would be overlooked and the patiens would therefore be exposed to a greater risk of developing serious late complications. Therefore we can conclude that there is an obvious need for active search of coeliac disease in all first-degree relatives of coeliac disease patients.
Summary     Izhodišča: Celiakija je danes vse pogostejša. Posebej ogrožene pa so nekatere skupine ljudi. Ena od teh skupin so ožji družinski člani bolnikov s celiakijo, kjer se prevalenca bolezni giblje med 2% in 12%. Da bi natančneje opredelili prevalenco bolezni med ožjimi družinskimi člani, smo v raziskavo vključili 80 ožjih družinskih članov 31 bolnikov s celiakijo. Preiskovanci in metode: V serumu preiskovancev smo določili prisotnost antigliadinskih (AGA) in antiendomizijskih (EMA) protiteles. Preiskovancem z zvišanimi vrednostmi protiteles smo predlagali, da opravijo biopsijo sluznice tankega črevesa. V bioptih smo ocenili histološke spremembe, značilne za celiakijo. Število bolnikov s celiakijo med ožjimi družinskimi člani smo podali kot prevalenco bolezni na 100 preiskovancev. Rezultati: Pri desetih preiskovancih (12,5%) smo odkrili zvišana protitelesa AGA IgG, pri eni preiskovanki (1,25%) protitelesa EMA, pri treh preiskovancih (3,75 %) pa protitelesa AGA IgG in EMA hkrati. Na biopsijo črevesne sluznice je pristalo pet preiskovancev, diagnozo pa smo potrdili pri štirih, tako da znaša prevalenca celiakije med ožjimi družinskimi člani 5%. Zaključki: Prevalenca celiakije je med ožjimi družinskimi člani visoka, bolniki pa spadajo v skupino bolnikov z asimptomatsko oz. atipično celiakijo, kjer diagnoze brez aktivnega iskanja ne bi mogli postaviti. Ti bolniki so izpostavljeni večjemu tveganju za razvoj resnih zapletov bolezni. Prav zaradi tega je med ožjimi družinskimi člani potrebno celiakijo aktivno iskati.
