Abstract     The introduction contains a review of literature showing the epidemiology, clinical picture, treatment and natural course of oral cavity carcinoma. The paper compares the treatment of patients with intraoral carcinoma carried out at the Maribor Teaching Hospital Department of ORL and Cervicofacial Surgery with that at the Zagreb Clinic for Facial, Maxillomandibular and Oral Surgery. The patients were all treated within the same period of time (1975-1985), the distribution according to sex, age, smoking or alcohol consumption habits was the same, and they also lived under similar socio-economic conditions. In Maribor, the majority of patients (70%) were treated primarily by radiotherapy while in Zagreb the initial treatment was surgical (90%). Tumor localization and age had no effect on the survival rate while in Zagreb the female patients had a slightly better prognosis. According to TNM classification and the stage of the disease, only in the first stage treatment was the same in both institutions - namely surgical. The success of treatment was significantly better in primary surgical treatment carried out in Zagreb. In Maribor, the same results were obtained only in the first stage of disease when treatment was predominantly surgical, while in the higher stages the irradiated patients have a poorer rate of recovery. This difference increases with each higher stage. In Zagreb, survival (90% surgical treatment) is equal or even better than the best results found in literature. In Maribor, survival (70% treatment by radiotherapy) is poorer than described in literature. In both approaches distant metastases represent a rare cause of death. The results obtained point to the fact that the initial method of treatment should be surgical.
