Abstract     In our contribution we reported a clinical course, treatment and complication of a severe hematogenous osteomyelitis of the left femur with sepsis and a concurrent septic arthritis of the left hip. It developed as a result of unsuccessful antibiotic treatment of tonsillitis with scarlatinoformic exantema. We described diagnostical pitfalls, which were probably the result of lack of specific clinical signs and symptoms. The doctors in a local hospital could not confirm the diagnosis with objective diagnostic methods. 3,5-years-old girl was after eleven days after admittion to the local hospital, because of the severe sepsis, urgently transferd in Department of orthopaedic surgery in Ljubljana. On admittion clinical signs showed picture of either severe bacterial arthritis of the hip or osteomyelitis of the pertrohanteric femural region. Using ultrasonic and rentgenographical exam we could also not exactly determine a place of the septic osteomyelitis or artritis. Extended anterior surgical exploration of the left hip showed a large pertrohanteric saubperiostal abscess with a concurrent septic arthritis of the hip. Our intention is to notice the general practitioners and pediatricians, that a severe hip pain and hip contracture at a child , which arise in the course of an infection disease or after it, could not be the result of only a reactive synovitis of the hip.
