Abstract | An unrecognised and untreated paracetamol poisoning could be fatal for the patients. Initial clinical presentation of paracetamol poisoning is usually mild and non-specific (nausea and vomiting) and can be easily overlooked. In the article a 21-year-old woman is presented. She had been treated by carbamazepine because epilepsy for the last two years. She attempted suicide with 8,5 g of paracetamol and with unknown amount of carbamazepine. On admission she was unconscious and had seizures. The laboratory evaluation revealed initial hepatotoxcicity and toxic blood concentration of paracetamol (120 - 150 mg/l) and carbamazepine (40 mg/l. In spite of immediately treatment with N-acetilcisteine, she develops severe, fotunately reversibile, liver damage. Induction of hepatic P450 enzymes by carbamazepine treatment may enhance paracetamol metabolism and hepatotoxcity that resulted in early sings of hepatotoxcicity and severe liver damage. | |
