Abstract     Cardiac tamponade is a frequent complication of acute type-A aortic dissection (AAD); it is also leading couse of death in patients with AAD. Undiagnosed and untreated cardiac tamponade leads to obstructive shock and death. We report a case of female patient who presented with the history of severe chest pain and colapse and with the signs of obstructive shock and in whom rapid diagnostic work-up and treatment was performed. Combination of trans-thoracic echocardiography (TTE) and spiral CT of the thoracic aorta revealed cardiac tamponade and AAD as the couse of patient's condition. After therapeutic pericardiocenthesis and haemodynamic stabilization the patient was transferred to the Center of Intensive Internal Medicine (CIIM), Clinical Center Ljubljana. Surgery was performed imediately after admission, based on diagnostic information obtained by TTE and spiral CT. We emphasise the importance of rapid diagnostic work-up and discuss the value of the combination of TTE and spiral CT when AAD is suspected, especially in institutions where trans-esophageal echocardiography (TEE), magnetic resonance imaging (MRI) and emergency aortography are not available on 24-hour basis.
