Abstract     Prehospital assessment of the patient with a heart problem begins with receiving the emergency call. The emergency dispatcher must obtain all important data from caller ( short history of event and present state of the patient as well as precise location of the patient) and activate the EMS rescue team. Dispatcher also gives instruction to the calling person, what he or she can do before EMS rescue team arrives to the patient. Members of EMS rescue team are medical doctor, who is a team leader and two medical technicians. A team leader is directing the team, ensure the safety of team members and the patient, starts with assessing the patient and gives further instruction for managing the patient. The most common heart diseases, involved in EMS interventions are acute ischaemic heart diseases (acute myocardial infarct), arrhythmias (bradyarrhytmias and tachycardias) and congestive heart failure (acute pulmonary oedema). In case of cardiac arrest the EMS team must start immediately with basic and advanced life support techniques. After initial assessment of acute ill patient with heart disease team provide appropriate therapy and care for patient, including putting the patient in appropriate position, attachment of electrodes for defibrillation end external pacing, connecting the patient to an ECG monitor, administration of oxygen, intravenous access, application of drugs, checking all procedures that had been carried out and monitor the patient using pulse oximeter and ECG monitor. The patient should be closely observed for any deterioration of his state all the time. When the primary management of the patient at the spot is finished, the EMS team carries the patient to the ambulance and transport him to the hospital, continuing appropriate monitoring and observing the patient all the time until admission to the hospital. Doctor provides all necessary documentation abut the patient, the rest ot the team takes care for the equipment.
