Abstract     Despite the increased knowledge, major hemorrhages in obstetrics will never be completely abolished. The causes of hemorrhages are uterine atony, injurines to the birth canal, uterine rupture, retained parts of the placenta, placenta previa, abruption. Besides, hemorrhages accompany preeclampsia, HELLP syndrome, amniotic fluid embolism, sepsis, intrauterine death, and all conditions triggering disseminated intravascular coagulation (DIC). Hemorrhage may also be caused by idiopathic thrombocytopenia. We present the guidelines for deciding on the infusion of crystalloids, plasma expander or blood transfusion. The anesthesiologists role is to correctly estimate the blood loss and to decide on the mode of treatment. Over the last 10-15 years, the blood transfusion has become more restrictive because of the potential transmission of severe viral deseases.
