Abstract		Although the first cases of surgery of anterior skull base tumours were first published over 45 years ago, such surgery is still rarely performed in Slovenia. In the past, severe postoperative complications occurred frequently because of insufficient reconstruction and lack of antibiotic treatment. Another important reason for the infrequent surgical treatment of these tumours is that they were situated in "no man's land"; they were situated too low for neurosurgeons, and too high for ENT and maxillofacial surgeons. With the subcranial approach to the anterior skull base, ENT surgeons operate in an operative field they are very familiar with. All important structures are well visible, so "en block" tumour resection and reconstruction are possible in most cases. This approach is useful for the treatment of benign and malignant tumours of the anterior skull base, and deformations and fractures of the ethmoid region with liquorrhea. This paper presents four patients operated on using this method at the Otorhinolaryngology and Cervicofacial Surgery Department of the Maribor General Hospital.
Summary		Čeprav je od prvih opisanih operacij tumorjev sprednje lobanjske baze preteklo več kot 45 let, so pri nas takšne operacije zelo redke. Zaradi slabše antibiotične zaščite v preteklosti in nezadostne obnovitve sprednje lobanjske baze so bili hudi zapleti pogosti. Drugi, verjetno še pomembnejši vzrok za redkejše operacije sprednje lobanjske baze je v tem, da je to "nikogaršnje" področje: za nevrokirurga je prenizko; za otorinolaringologa in maksilofacialnega kirurga previsoko. S subkranialnim pristopom pa se otorinolaringolog nahaja skoraj ves čas v njemu znanem področju. Ta pristop omogoča dober pregled nad vsemi pomembnimi strukturami in v večini primerov odstranitev tumorja "en block" ter daje možnost dobre obnovitve sprednje lobanjske baze. Sam pristop je uporaben pri operacijah benignih in malignih tumorjev, ki se nahajajo v bližini sprednje lobanjske baze, deformacijah in pri poškodbah sitkinega predela z večjim iztekanjem likvorja. V članku je opisana operativna tehnika in prikazan potek zdravljenja pri 4 bolnikih, ki smo jih operirali na Oddelku za otorinolaringologijo in cervikofacialno kirurgijo Splošne bolnišnice Maribor.
