Abstract     After extensive myocardial infarction left ventricular aneurysm represents systolic convexity of left ventrical with the diminished trabecular structures and with expressed border line between viable and fibrotic tissue. It causes malignant rhythm disturbances with high incidence of sudden death. Medical treatment is less effective in the comparison with surgical therapy. Conventional surgical therapy represents aneurysmectomy with longitudinal incision of aneurysmatic sack and double suture closure with teflon felt. Because of the severe reduction of left ventricular volume and disfunction of papillary muscles after resection of aneurysmatic sack the success of this procedure is questionable. The mortality rate is relatively high. New surgical procedure - endoaneurysmorraphy enables the improvement of left ventricular function in the comparison with classical surgical therapy. Left ventricular dimension is stabilized and the ejection fraction is improved by the implantation of dacron patch without resection of aneurysmatic sack. Tha purpose of the study was to evaluate the success of the new surgical procedure- endoaneurysmorraphy. The global and regional contraction of left ventricle before and after the implantation of dacron patch was compared. Clinical and diagnostical data were evaluated leading to the decision to choose the proper therapy for the patients with post infaction aneurysm of left ventricle. Regional left ventricular function was evaluated according to preoperative contrast ventriculagraphy and postoperative radionuclide ventriculography with technetium 99. The measurements of svstolic and dyastolic regional half diameter and regional surfaces of left ventricle before and after operation werc compared. The results of contrast ventriculography and radionuclide ventriculography were analysed with planimetric method by the use of coordinating system (the method of shortening the semi axis).
