Abstract     Proinflammatory cytokines, their antagonists, soluble receptors and soluble adhesion molecules (AM) all may be useful parameters of the activity of ANCA-positive vasculitis and lupus nephritis. Plasma levels of IL-1-beta, IL-1ra, IL-6, IL-8, TNF, IL-6R, TNF-R II, ICAM-1 and VCAM-1 were measured by ELISA in 18 patients (pts) with ANCA-positive renal vasculitis (12 active - ANCA-A, 6 in remission ANCA-R), 9 pts with active lupus nephritis (LN) and 9 healthy subjects (Co). Fractional excretion (FE) of selected cytokines and AM was also calculated. Pts with LN had in comparison with Co increased plasma ICAM-1 (183.22 +- 40.75 vs. 108.16 +- 28.75 ng/ml, p < 0.01), VCAM-1 (1537.88 +- 677.09 vs. 692.26 +- 133.25 ng/ml, p < 0.01), IL-6 (19.11 +- 1 1.04 vs. 2.70 +- 0.84 pg/ml, p < 0.01), IL-8 (44.67 +- 12.19 vs. 31.44 +- 5.08 pg/ml, p < 0.05), soluble TNF-R II (8.39 +- 3.85 vs. 1.86 +- 0.31 ng/ml, p < 0.01), soluble IL.-6 receptor (43.11 +- 14.0,2 vs. 29.9 +- 5.65 ng/ml, p < 0.05) and increased ratio between plasma IL-6 and IL-6R (0.48 +- 0.37 x 10-3 vs. 0.09 +- 0.03 x 10-3, p < 0.05). Pts with LN had in comparison with co increased urinary excretion of IL-8 (35.78 +- 34.37 vs. 12.46 +- 15.56 (g/mol creat, p < 0.05), TNF (2.81 +- 1.24 vs. 0.98 +- 0.15 (g/mol creat, p < 0.01) and VCAM-1 (48.98 t 49.50 vs. 2.92 +- 4.06 mg/mol creat, p < 0.05) and increased FE of IL-8 (7.93 +- 6.64 vs. 2.56 +- 3.20%, p < 0.05), VCAM-1 (0.27 +- 0.16 vs. 0.03 +- 0.05%, p < 0.01), but not of TNF.
