Abstract     A heart disease is extremly important at patients with renal replacement therapy. In the groups of patients with chronic renal failure are mortality and morbidity strongly dependent on cardiovascular derrarangements. Manifestationes of cardiovascular remodelling starts early in the course of chronic renal insufficiency and from this reason is extremly important early diagnostics and therapy. Arterial hypertension, anemia, effect of arterio-venous fistula, rapid change of extracelular volume and secondary hyperparathyroidism are most important risk factors. Uremic milleaue is the independent risk factor and is influenced with mode, quality and quantity of the renal replacement therapy. A hypertrophy of the left ventricle is also independent risk factor and this type of remodelling starts at the begining of renal disease. A parathyroid hormone is permissive factor for fibrosis or inadequate hypertrophy and from this reason is extremly important control of secondary hyperparathyroidism. Ischemic heart disease is twentyfold more often than at generally population. Early diagnostics, therapy, primary and secondary prevention of cardiovascular system is the key for patients with end-stage renal diseases.
