Title     Morphological characteristics of vessels established by duplex sonography at 116 patients with end-stage renal disease (ESRD)

Abstract     Background. For successful arteriovenous fistula creation and maturation good quality of vessels is needed. Because of many reasons the vessels of pts with ESRD could be damaged. Methods. In this study vessels were examined clinically and by duplex sonography to evaluate the quality of vessels in group of pts with ESRD. Those results were compared with findings during surgery. Results. Out of 116 pts observed 62 were female and 54 male, average age was 51.4 yrs (range 15 to 80 yrs), 64 (55.2 %) of them were on hemodialysis for mean 24.5 months, mean serum creatinine level in the rest of 52 pts was 775.8 micromol/I. By clinical examination vein visibility was good at 54 (46%) of pts, inadequate at 24 (20.7%) and veins were not found at 38 (32.8%) of pts. In this 62 pts veins were examined by duplex sonography. Useful veins were confirmed at 48 (77.4%) pts and phlebograpy was needed at 14 (22.6%) of them. Palpability of the radial artery pulse was good at 53 (45.7%) pts, weak at 43 (37%) and arteries were not palpable at 20 (17.3%) pts. Mean internal arterial diameter was 0.244 +- 0.08 cm (range 0.1 to 0.4 cm), at 62 females pts diameter was 0.212 +- 0.03 cm, and at 54 males diameter was 0.253 +- 0.07 cm (p<.01). Mean thickness of the artery wall was 0.037 +- 0.02 cm. In group of 60 pts older than 55 yrs it was 0.048 +- 0.018 cm while in remaining 56 pts under the age 55 yrs it was 0.024 +- 0.014 (p<.01). Correlation between the age of pts and the thickness of wall was good (r=0.57;p<.01). Normal structure of the arterial wall was found in 32 (27.5%), initial changes with hyperechogenicity in 38 (32.7%), diffuse changes with rough arterial lumen 31 (26.7%) and calcification of the wall in 15 (12.9%) of 116 examined pts. Correlation coefficient by Spearman for results of duplex imaging and direct evaluation during surgery was 0.82 (p<.001). (Abstract truncated at 2000 characters.)
